How CareConnect Health handles your information when you use careconnecthealth.com.
This policy explains how CareConnect Health (“CareConnect,” “we,” “us”) handles personal information collected through careconnecthealth.com (the “website”). It applies to visitors to this website, including families, caregivers, doctors, practices, community organizations, home care agencies and partners.
It does not describe how we handle health information as part of delivering care. If you take part in a care program, the information we collect for your care is covered by the consent forms and privacy notices you receive at enrollment, and those documents control for that information.
The contact form is for general enquiries. Do not enter diagnoses, medications, Medicare numbers, Social Security numbers or other medical or sensitive details in the form or in emails to us. Standard email and web forms are not a secure way to share health information. A member of our team will explain how to share anything of that kind safely.
Information you give us. When you use the contact form or email us, we collect:
Information collected automatically. When you visit, our website host and its tools may record your IP address, browser and device type, the pages you view, the website that referred you, and the date and time of your visit. This data is used to keep the site working and secure and to understand how it is used.
Information that stays in your browser. The eligibility checker and funding estimator on the Medicare GUIDE page run on your device. Your answers are not sent to us or stored by us. We also save your cookie choice in your browser (see Cookies below).
We do not sell your personal information.
A banner on our website asks for your choice about cookies and similar technologies, which fall into these groups:
Choosing Accept or Deny saves your choice in your browser. You can change it at any time with the “Cookie settings” link in the footer of every page. You can also block or delete cookies in your browser settings, though some features may then not work as intended.
We keep enquiries and contact details for as long as we need them to respond and to keep reasonable business records, and for any longer period the law requires. When information is no longer needed, we delete or de-identify it. Server and analytics data are kept for the periods set by our hosting and analytics providers.
We use reasonable administrative, technical and organizational safeguards to protect personal information, including secure (HTTPS) connections and limiting access to people who need it. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
You can ask us to tell you what personal information we hold about you, to correct it, or to delete it, and you can ask us to stop contacting you. Depending on where you live (for example, California and some other U.S. states), you may have additional rights, such as to know what we collect, to delete or correct it, to opt out of sale or sharing for targeted advertising, and not to be treated unfairly for using these rights.
To make a request, email contactus@careconnecthealth.com. We may need to confirm your identity first, and we will respond within the time the law allows. If you are not satisfied with our response, you may contact your state attorney general or other privacy regulator.
This website is for adults. It is not directed to children under 13, and we do not knowingly collect personal information from children. If you believe a child has given us information, email us and we will delete it.
Our pages may link to other sites, such as government program pages. Those sites have their own privacy practices, and this policy does not cover them.
We may update this policy from time to time. The “Last updated” date below shows when it last changed. If we make a material change, we will post the updated policy here.
